About The Borealis PCN

The Borealis PCN was formed to provide timely and accessible primary health care services to the patients in our communities. The Borealis Primary Care Network was formed by the merging of the Peace Region and Saddle Hills PCNs on April 1, 2023. Primary Care Networks have been active in the region since 2007.

Position Summary

Reporting to the Social Worker, the Referral Coordinator works with patients and multidisciplinary health care teams to arrange and schedule referral appointments to Borealis Primary Care Network (PCN) services.

What you will do...
  • Assist PCN programs by coordinating referral appointments and working with insurance companies and care providers to share essential patient information
  • Maintaining patient records
  • Develop relationships with various community health resources to support patients requiring treatment and referrals
  • Reviewing referrals to PCN programs and scheduling appointments using designated software and other computer applications
  • Ensure referral forms have been completed and that relevant information is included with referrals
  • Communicate receipt of referrals back to referring health practitioners
  • Administration-related procedures which are program specific:
    • Schedule, confirm and book patient appointments and referrals
    • Maintain patient records accordingly
    • Utilize EMR for appointments and patient information
  • Provide general administrative support where needed
What you need to succeed...
  • High school diploma
  • Completion of a post-secondary Medical Office Assistant program
  • A minimum of two (2) years related administrative experience in a medical office required
  • Patient advocacy experience is an asset
  • Demonstrates ability to look for innovative solutions when managing tasks
  • Strong oral, written and analytical skills; self-directed and accountable
  • Ability to maintain patient privacy and confidentiality
  • Attention to details and problem-solving skills
  • Able to work independently and demonstrates ability to be proactive in completing duties
  • Strong organizational and time management skills, including the ability to handle multiple assignments and tasks while meeting deadlines
  • Ability to remain calm in difficult situations and deal effectively with those who are in distress
  • Ability to maintain effective working relations with employees and customers
  • Ability to maintain accuracy while multi-tasking or working under pressure
  • Ability to collaborate with other multi-disciplinary team members
  • Ability to follow directions closely and accurately
  • Working knowledge of office equipment, like printers and fax machines
  • Understanding of EMR software is an asset
  • Proficiency in Microsoft Office (Excel, PowerPoint, Word, Outlook, and Teams in particular)
