In order to improve timely access to health care for patients, the PCN is offering enhanced care for complex health issues and chronic disease management.

The Chronic Disease Management initiative has programs available in both the Sexsmith Medical Clinic and the Central Peace Health Centre.

The Chronis Disease Management program will relaunch in Beaverlodge May 2023.
