About The PCN

The Borealis PCN was formed to provide timely and accessible primary health care services to the patients in our communities. The Borealis Primary Care Network was formed by the merging of the Peace Region and Saddle Hills PCNs on April 1, 2023. Primary Care Networks have been active in the region since 2007.

Position Summary

The Licensed Practical Nurse (LPN) works collaboratively with the multidisciplinary team to enhance the delivery of primary care services to patients. The role will work within the scope of practice to participate in the provision of care to patients with a variety of clinical conditions. This posting is for two 0.5 FTE permanent positions.

What you will do...
  • Works within the scope of practice as part of the clinic team to provide support to physicians and patients in day-to-day clinical activities
  • Perform a variety of nursing duties to provide consistent high-quality care to meet the needs of the patients through the lifespan
  • Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and interdisciplinary team
  • Provide short term interventions as needed
  • Provide relevant patient education to prevent and manage clinical conditions
  • Ensure the maintenance of accurate records in line with practice policy
  • Assist patients in navigating through the health system and provide direction in accessing local and regional health programs
  • Communicate with patients to determine the next appropriate health services required based on presenting concerns and test results
  • Support patients in facilitating appropriate referrals to community resources and programs
  • Participate in the evaluation of the PCN clinic and its services
  • Assist in planning, developing, and monitoring for services provided
  • Assist in the development of policies and protocols for clinic programs
  • Assist in setting up health promotion programs relevant to the needs of the practice population
  • Assist in developing and implementing patient self-management programs
  • Assist in the implementation of strategies for improving the quality and provision of care for the practice population
What you need to succeed...
  • Successful completion of an accredited Licenced Practical Nurse diploma program
  • Five years or more of related LPN experience
  • Current CLPNA Registration in good standing and current BLS
  • Clinical experience in primary care nursing, after-hours or primary care practice settings an asset
  • Clinical skills in primary care nursing
  • Knowledge of nursing processes and practices, including intervention strategies and therapies
  • Strong oral, written and analytical skills; self-directed and accountable and high level of perceptiveness
  • Ability to maintain patient privacy and confidentiality
  • Attention to details and problem-solving and critical thinking skills
  • Able to work independently and demonstrates ability to be proactive in completing duties
  • Strong organizational and time management skills, including the ability to handle multiple assignments and tasks while meeting deadlines
  • Ability to remain calm in difficult situations and deal effectively with those who are in distress
  • Ability to maintain effective working relations with employees and customers
  • Ability to maintain accuracy while multi-tasking or working under pressure
  • Ability to collaborate with other multi-disciplinary team members
  • Understanding of EMR software is an asset
