The area covered by the Borealis PCN includes the towns of Peace River, Manning, Beaverlodge, Hythe, Wembley, Sexsmith, and Spirit River in northwest Alberta. People in the region enjoy, museums, arts, parks, and various recreation facilities. The region is also home to a wide array of wildlife and big game including moose, white-tailed deer, mule deer, and elk.

About The PCN

The Borealis PCN was formed to provide timely and accessible primary health care services to the patients in our communities. The Borealis Primary Care Network was formed by the merging of the Peace Region and Saddle Hills PCNs on April 1, 2023. Primary Care Networks have been active in the region since 2007.

Position Summary

Peace River and Saddle Hills Primary Care Networks are merging to create the Borealis Primary Care Network generating new and exciting opportunities!
The Nurse Practitioner (NP) is responsible for working collaboratively with the organization’s leadership team to conceptualize and develop evaluation and performance measurement frameworks and support the nursing professionals in their roles.

What you will do...
  • Lead and manage the clinical team consisting of nursing professionals and medical office assistants
  • Develop the skills and abilities of the nursing professionals and medical office assistants
  • Provide restricted activities and care within the full scope of practice as defined under the Health Professions Act (HPA)
  • Initiate/recommend appropriate investigations/referrals
  • Perform comprehensive assessment, diagnosis (including the ordering and interpretation of tests), intervention (including the prescribing of medications), management, referral, and evaluation of patient/client care across the continuum of health care services
  • Assist in the delivery of disease management and health promotion programs and provide appropriate care and treatment according to currently accepted evidence-based practice
  • Undertake disease management, incorporating monitoring progress, response to treatment, and promoting the preventive aspects of patient care
  • Ensure accurate, objective and timely clinical documentation in the appropriate medical record
  • Work collaboratively in a patient centered, integrated team-based practice within a primary care setting
  • Participate in the care of individual patients, practice populations and associated communities
  • Assist physicians and clinical staff on a as needed
  • Provide coverage for physicians as appropriate
What you need to succeed...
  • Master of Nursing, with a Family Nurse Practitioner focus
  • Must hold current registration with the College and Association of Registered Nurses of Alberta (CARNA)
  • A minimum of 8-10 years nursing experience and 2 years as a Nurse Practitioner is required
  • Primary Care nursing experience is required
  • Current Basic Life Support (BLS) for a healthcare provider
  • High levels of integrity and professional behaviour
  • Ability to maintain patient privacy and confidentiality
  • Attention to details and problem-solving and critical thinking skills
  • Able to work independently and demonstrates ability to be proactive in completing duties
  • Ability to delegate effectively and provide leadership, advice, and consultation to others
  • Ability to lead change including quality improvement initiatives
  • Strong organizational and time management skills, including the ability to handle multiple assignments and tasks while meeting deadlines
  • Understanding of EMR software is an asset
  • Computer skills related to Microsoft Office suite
  • Valid drivers license and access to a vehicle is required
  • Understanding of EMR software is an asset
